OBJECTIVES/GOALS: Renal fibrosis is a critical pathophysiological event in chronic kidney diseases. Our goal is to determine the ability of dual-inhibitor of transforming growth factor beta receptor 1 (TGFα²R1) and mitogen-activated protein kinase kinase kinase kinase 4 (MAP4K4), TK850, on reducing kidney fibrosis. METHODS/STUDY POPULATION: To test the renal anti-fibrotic action ofdual TK850,8-10-week-old male and female C57BL/6mice with unilateral ureteral obstruction (UUO) induced kidney fibrosis were used. Mice were separated into 3 groups: group 1 contained mice that had UUO surgery (UUO control), group 2 contained mice prophylactically treated with TK850 thatstarted 7 days prior to UUO(UUO-P,20 mpk/d/ip), and group 3 contained mice interventionally treated with TK850 that started3 days after UUO(UUO-I,20 mpk/d/ip). Ten days following UUO the kidneys and blood were collected for analysis. Renal fibrosis was assessed from hydroxyproline content (measure of collagen)and histological collagen analysis using Picrosirius red stain. RESULTS/ANTICIPATED RESULTS: Renal hydroxyproline was increased equally in the UUO kidney of male (5.4 ± 0.41 µg/10mg, n=5) and female mice (5.5 ± 0.50 µg/10mg, n=5) compared to the contralateral control kidney (2.9 ± 0.14 µg/10mg, n=10). TK850 treatment in UUO-P mice (n=10, 3.4 ± 0.24 µg/10mg) and UUO-I mice (4.30 ± 0.20 µg/10mg, n=10) had significantly reduced hydroxyproline levels. Histopathological evaluation revealed that kidney injury increased collagen deposition in the UUO kidney (17.1 ± 0.43% collagen positive area, n=10) compared to the control kidney (2.0 ± 0.23%, n=10). TK850 treatment in UUO-P mice significantly attenuated collagen deposition (10.5 ± 0.38%, n=10), while UUO-I had significantly reduced collagen deposition as well (13.1 ± 0.25%, n=10). DISCUSSION/SIGNIFICANCE: Taken together, these results validate the dual TGFβR1/MAP4K4 inhibitor, TK850 as a potential therapeutic to mitigate renal fibrosis and supports the emergence of a combinational pharmacotherapeutic approach for multi-factorial kidney diseases.

Objective:
There is increasing focus on effective preventative interventions applicable at the population scale such as through technology and web-based approaches. We aimed to reduce cognitive decline with ageing using an online package of interventions delivered intensively for 12 months followed by monthly boosters for 24 months.
Methods:Invitations were sent to people aged 55-77 years from the 45 and Up study, a population-based cohort study of one in ten people aged 45 years and older in New South Wales, Australia (n=267,000). Participants were required to be eligible for at least two of four modules addressing physical inactivity and associated health risks (Physical Activity), adherence to a Mediterranean-type diet and health risks associated with poor nutrition (Nutrition), cognitive activity (Brain Training) and mental well-being (Peace of Mind). Participants received modules based on their risks, with 1:1 randomized allocation to active personalised coaching modules (intervention) or static information-based modules (control). The primary outcome was change in an online combined multi-domain cognitive score measured using COGSTATE and Cambridge Brain Sciences tests using intention to treat analysis. Secondary outcomes included specific cognitive domain and ANU-ADRI risk scores.
Results:From 96,418 invitations, 14,064 (14%) consented; 11,026 (11%) were eligible; and 6,104 (6%) completed all 10 baseline assessments. Over three years there was a significantly greater improvement in the global composite cognition z-score in the intervention group, ES = 0.106 (p<0.001). Significant benefits were also found in complex attention, executive function and learning and memory (all p<0.001), as well as on a validated dementia risk instrument (p=0.007).
Conclusion:An online platform that tailored physical activity, nutrition, brain training, depression and anxiety interventions to an individual’s risk factor profile over three years significantly delayed cognitive decline in older adults. This platform is scalable and if rolled out at a population level may help reduce the prevalence of dementia globally.

Objective
ENT specialty trainees are invited to national bootcamps to prepare for critical events to which they may have never been exposed. Here, we evaluate the frequency of out-of-hours ENT operations and the level of supervision provided to inform aspiring trainees and the national bootcamps.
MethodsInformation on all ENT operations performed out-of-hours was prospectively recorded by trainees in Kent, Surrey and Sussex over seven months.
ResultsThere was a 100 per cent response rate. The trainee was the most senior surgeon present for 48.4 per cent of out-of-hours operations. Four of the six most frequently performed out-of-hours operations are not included in the ENT indicator procedures, and two are not included in the national ENT bootcamps.
ConclusionTrainees should be aware of the most common procedures they may be expected to perform out-of-hours. Training in these should be provided prior to the commencement of specialty training. The audit design can be replicated across surgical specialties.

Summary
What does the future of making media hold? This is the question that we put to four media scholars and keen observers of media industries and production worldwide. We conducted interviews with them on the basis of key questions and insights gained during the compiling of this book. They provide perspectives from North America (Henry Jenkins), Latin America (Elizabeth Saad Corrẽa), Southeast Asia (Anthony Fung), and Africa (Tanja Bosch).
Making media: Production
Q1: Considering the various trends and developments shaping media production, we argue that processes of disruption and consolidation co-exist within the media industries when it comes to media production practices. Organizations and firms can choose to experiment and pioneer to develop new products and services, or opt to double down on existing ways of doing things. What key trends do you observe in this context, and what about this trend makes you hopeful about the prospects for media makers?
ELIZABETH SAAD CORRẼA: The co-existence of disrupting processes and the consolidation of existing ones provides balance to media production, considering the constant IT innovations. It is a trend that is almost mandatory within the media industry. The choice between pioneering or just preserving familiar processes depends on how the organization is engaged in an innovative strategic profile. It also depends on different cultural, social, and economic aspects of each media company. Visual/video storytelling and longform journalism using different visual and graphic resources are evolving trends in the Brazilian media industry. We can find interesting examples, such as UOL Tab, with a specialized team working on developing longform stories, and Nexo Jornal that uses infographics and data journalism to explore news contextualization.
ANTHONY FUNG: I see four possible prospects. First, digitalization. Digital platforms are now ‘the media’, a new notion that we haven't seen before. Second, under digitalization, cultural production, curation, distribution, and sharing (as these platforms are all social media or carry the function of social media) may be newer concepts offering new opportunities.

Diketopyrrolopyrrole (DPP) is a critically important building block that has gained importance in the organic electronics community because of its wide applicability in various devices. In this work, the thiophene flanked DPP moiety attached to alkyl chains of various lengths (this includes straight octyl and branched ethyl hexyl units) has been used as the monomer for electropolymerization. This paper focuses on the study of optical, thermal, solid state ordering and electrochemical properties of these electron deficient monomers using various characterization techniques such as UV–Vis spectrometry (UV), photo-luminescence spectroscopy (PL), differential scanning calorimetry (DSC), thermal gravimetric analysis (TGA), X-ray diffraction (XRD), cyclic voltammetry (CV), as well as ab initio modeling. These monomers exhibit broad absorption spectra from the ultraviolet (280–400 nm) to visible (400–600 nm) regions and emission spectra between 560 and 610 nm. The band gaps of these monomers were calculated to be in the range of 2.00–2.20 eV. These monomers were electropolymerized by scanning the potential between −0.5 and 2.0 V versus ferrocene for up to 50 cycles on a glassy carbon electrode.

Electropolymerization is a promising approach to produce thin films of active organic conjugated materials on a desired conducting substrate. In this work, an electropolymerization study has been carried out on two diketopyrrolopyrrole (DPP)-based monomers 2,5-bis(2-butyloctyl)-3,6-di(furan-2-yl)pyrrolo[3,4-c]pyrrole-1,4(2H,5H)-dione (BO-DPPF) and 2,5-bis(2-butyloctyl)-3,6-di(thiophen-2-yl)pyrrolo[3,4-c]pyrrole-1,4(2H,5H)-dione (BO-DPPT). These monomers consist of thiophene and furan heterocyclic moieties attached to a DPP core with a common solubilizing alkyl chain (butyl-octyl). The properties of these monomers were analyzed via differential scanning calorimetry, thermogravimetric analysis, UV–Vis spectrometry (UV) and photoluminescence. Cyclic voltammetry (CV) studies indicate the presence of irreversible oxidation and reduction reactions. The electropolymerization of BO-DPPF and BO-DPPT electron-deficient monomers to form polymer films on a glassy carbon electrode is achieved by applying a potential between −2 V and 2 V versus ferrocene for up to 50 cycles. The properties of the polymers were investigated using the cyclic voltammetry (CV) technique.
Recovering function and surviving treatments are primary motivators for health behavior change in patients with head and neck cancer: Qualitative focus group study

Objective:
Against medical advice, head and neck cancer (HNC) patients have been shown to continue to smoke and misuse alcohol post-diagnosis and treatment. This study aimed to better understand the barriers to and facilitators of health behavior change (HBC) in HNC patients.
Method:We conducted nine focus groups following a standard protocol. Eligible patients were diagnosed less than three years previously with a primary HNC and selected using maximum variability sampling (gender, age, cancer stage, smoking, and alcohol misuse). Thematic analysis was conducted using NVivo 10 software.
Results:Participants were mostly men (79%), 65 years of age (SD = 10.1), and married/common-law (52%, n = 15). Mean time from diagnosis was 19 months (SD = 12.3, range = 5.0–44.5), and most had advanced cancer (65.5%, n = 19). Participants provided a larger than anticipated definition of health behaviors, encompassing both traditional (smoking, drinking, diet, exercise, UV protection) and HNC-related (e.g., dental hygiene, skin care, speech exercises, using a PEG, gaining weight). The main emerging theme was patient engagement, that is, being proactive in rehabilitation, informed by the medical team, optimistic, flexible, and seeking support when needed. Patients were primarily motivated to stay proactive and engage in positive health behaviors in order to return to normal life and reclaim function, rather than to prevent a cancer recurrence. Barriers to patient engagement included emotional aspects (e.g., anxiety, depression, trauma, demoralization), symptoms (e.g., fatigue, pain), lack of information about HBC, and healthcare providers' authoritarian approach in counseling on HBC. We found some commonalities in barriers and facilitators according to behavior type (i.e., smoking/drinking/UV protection vs. diet/exercise).
Significance of Results:This study underlines the key challenges in addressing health behaviors in head and neck oncology, including treatment-related functional impairments, symptom burden, and the disease's emotional toll. This delicate context requires health promotion strategies involving close rehabilitative support from a multidisciplinary team attentive to the many struggles of patients both during treatments and in the longer-term recovery period. Health promotion in HNC should be integrated into routine clinical care and target both traditional and HNC-related behaviors, emphasizing emotional and functional rehabilitation as key components.

Cognitive factors, such as the accessibility of schematic categories, have been shown to bias interpretation of information about other's behaviour. In this study, the effects of diagnostic category accessibility on subjects' causal attributions for an interviewee's behaviour were investigated. In order to activate a specific diagnostic category, making it more accessible for information encoding, mental health students completed a short test. Half answered questions about victims of rape attacks; the other half answered questions about the diagnosis of histrionic personality disorder. All subjects then rated their attribution for a female interviewee's non-verbal behaviour along four dimensions: Internality, Stability, Globality, and Controllability. Results indicated an interaction between type of course and experimental condition. Psychology undergraduate students in the Rape Victim condition tended to make more stable and global attributions than did Psychiatric Nursing and Masters of Psychological Medicine students. In addition, a significant main effect was found on the Controllability dimension. The subjects in the Personality Disorder condition rated the cause as significantly more controllable than did the subjects in the Rape Victim condition. Implications of the results are discussed.

Purpose
To evaluate the efficacy of a Belly Board immobilisation device for rectal cancer patients.
Materials and methodsA randomised trial in patients receiving neo-adjuvant chemoradiation for rectal carcinoma was established. Patients were treated, prone with control arm, according to standard departmental protocol and experimental arm with the use of a Belly Board. All treatments were planned using a three-field technique. The primary endpoints were reproducibility and irradiated small bowel volume. Questionnaires were used to assess secondary endpoints of patient comfort, ease of set-up and acute toxicities.
ResultsPre-planned interim analysis was performed after recruiting 30 patients. In all, 348 portal images were analysed retrospectively. Around 8 out of 12 parameters measuring set-up reproducibility were in favour of the Belly Board arm. Random error in the anterior–posterior direction was improved and statistically significant in the experimental arm (95% CI; p≤0·05). Small bowel V15 was significantly lower in the Belly Board position (mean V15=14·5%) compared with the standard position (mean V15=21·4%), paired t-test 95% CI; p=0·035. Also, patients’ comfort satisfaction was greater in the Belly Board arm.
ConclusionsSet-up reproducibility, small bowel V15, patient comfort and satisfaction were all significantly improved by the use of the Belly Board.

Background: Disagreement currently exists regarding the definition of aortic dextroposition. It is suggested that the term be used interchangeably with aortic overriding, along with suggestions that the aortic valve overrides in the normal heart. The dextroposed aorta, however, does not always override the crest of the muscular ventricular septum. It is incorrect to argue that the normal aortic valve overrides. It is the cavity of the right aortic valvar sinus, rather than the valvar orifice, that sits above the muscular septum when the septum itself is intact. Therefore, to circumvent these difficulties, those using the term “dextroposition” find it necessary to distinguish “true” as opposed to “false” categories. The problems arise because “dextroposition” is remarkably ill-suited as an alternative term for aortic valvar overriding. Methods and Results: In this review, combining developmental, morphologic, and clinical data, we show how aortic overriding is best considered on the basis of biventricular connection of the aortic root in the setting of deficient ventricular septation. When analysed in this manner, it becomes an easy matter to distinguish between one-to-one and double outlet ventriculo-arterial connections. Appreciation of these features emphasises the different spatial alignments of interventricular communications as opposed to the plane of deficient ventricular septation. The concept of overriding is applicable not only to biventricular connection of the aortic root, but also the pulmonary and common arterial roots. Conclusions: The diagnostic techniques now available to the paediatric cardiologist illustrate the features of arterial valvar overriding with exquisite accuracy, informing the discussions now required for optimal decision making.

Objectives:
No study systematically has investigated the supportive care needs of general head and neck cancer patients using validated measures. These needs include physical and daily living needs, health system and information needs, patient care and support needs, psychological needs, and sexuality needs. Identifying the unmet needs of head and neck cancer patients is a necessary first step to improving the care we provide to patients seen in our head and neck oncology clinics. It is recommended as the first step in intervention development in the Pan-Canadian Clinical Practice Guideline of the Canadian Partnership Against Cancer (see Howell, 2009). This study aimed to identify: (1) met and unmet supportive care needs of head and neck cancer patients, and (2) variability in needs according to demographics, disease variables, level of distress, and quality-of-life domains.
Methods:Participants were recruited from the otolaryngology–head and neck surgery clinics of two university teaching hospitals. Self-administered questionnaires included sociodemographic and medical questions, as well as validated measures such as the Supportive Care Needs Survey–Short Form (SCNS-SF34), the Hospital Anxiety and Depression Scale (HADS), and the Functional Assessment of Cancer Therapy–General (FACT-G) and Head and Neck Module (FACT-H&N) (quality of life measures).
Results:One hundred and twenty-seven patients participated in the survey. 68% of them experienced unmet needs, and 25% revealed a clinically significant distress level on the HADS. The highest unmet needs were psychological (7 of top 10 needs). A multiple linear regression indicated a higher level of overall unmet needs when patients were divorced, had a high level of anxiety (HADS subscale), were in poor physical condition, or had a diminished emotional quality of life (FACT-G subscales).
Significance of results:The results of this study highlight the overwhelming presence of unmet psychological needs in head and neck cancer patients and underline the importance of implementing interventions to address these areas perceived by patients as important. In line with hospital resource allocation and cost-effectiveness, one may also contemplate screening patients for high levels of anxiety, as well as target patients who are divorced and present low levels of physical well-being, as these patients may have more overall needs to be met.

Executive Summary
The severity of the impacts of extreme and non-extreme weather and climate events depends strongly on the level of vulnerability and exposure to these events (high confidence). [2.2.1, 2.3, 2.5] Trends in vulnerability and exposure are major drivers of changes in disaster risk, and of impacts when risk is realized (high confidence). [2.5] Understanding the multi-faceted nature of vulnerability and exposure is a prerequisite for determining how weather and climate events contribute to the occurrence of disasters, and for designing and implementing effective adaptation and disaster risk management strategies. [2.2, 2.6]
Vulnerability and exposure are dynamic, varying across temporal and spatial scales, and depend on economic, social, geographic, demographic, cultural, institutional, governance, and environmental factors (high confidence). [2.2, 2.3, 2.5] Individuals and communities are differentially exposed and vulnerable and this is based on factors such as wealth, education, race/ethnicity/religion, gender, age, class/caste, disability, and health status. [2.5] Lack of resilience and capacity to anticipate, cope with, and adapt to extremes and change are important causal factors of vulnerability. [2.4]
Extreme and non-extreme weather and climate events also affect vulnerability to future extreme events, by modifying the resilience, coping, and adaptive capacity of communities, societies, or social-ecological systems affected by such events (high confidence). [2.4.3] At the far end of the spectrum – low-probability, highintensity events – the intensity of extreme climate and weather events and exposure to them tend to be more pervasive in explaining disaster loss than vulnerability in explaining the level of impact.
A barium-rich binary central star in Abell 70

We have found the central star of Abell 70 (PN G038.1–25.4, hereafter A 70) to be a binary consisting of a G8 IV-V secondary and a hot white dwarf. The secondary shows enhanced Ba II and Sr II features, firmly classifying it as a barium star. The nebula is found to have Type-I chemical abundances with helium and nitrogen enrichment, which combined with future abundance studies of the central star, will establish A 70 as a unique laboratory for studying s-process AGB nucleosynthesis.
